The initial review of an item being offered to us for purchase is via detailed photos of the item, the paperwork and box that come with it, prior to receipt. Many counterfeits are rejected at this step. Reasons for rejection at this initial stage can include, but are not limited to:

Part of our authentication process is to request any brand paperwork and receipts from the seller. As such, we typically get brand certificates and/or receipts for approximately half of the items we buy from Cartier and Van Cleef & Arpels (most other jewelry brands do not provide such certificates of authenticity).
Among other methodologies, myGemma uses a Thermo Scientific Precious Metals Analyzer to test metal purity for compliance. For example, this is used on the hardware of Hermès Birkins and Cartier LOVE bracelets to ensure the metal compositions match the brands' standards.
In addition, myGemma uses a UV lamp to check diamonds for fluorescence (which for some brands can be an indicator of a replica) and to check handbags for third-party retouching. myGemma does not accept handbags that have been "re-painted" to improve the condition.
